Högby sockens tidigast kända kyrkobyggnad anses ursprungligen vara från mitten av 1100-talet.Den var byggd av sandsten och bestod av ett långhus och kor.Eftersom ön hade ett utsatt läge för strandhugg och invasioner uppfördes under 1200-talet ett kraftigt försvarstorn i väster och en stor korbyggnad i öster. Då långhuset var mindre fick kyrkan karaktär av klövsadelkyrka. Vid södra sidan av långhuset uppfördes någon gång under medeltiden en tillbyggnad som rymde vapenhus och Sankt Ottos kapell . Korets norrsida hade också försetts med en sakristia. Under 1200-talet hade kyrkorummet välvts och blev på detta sätt en tvåskeppig hallkyrka [1] .
När den medeltida kyrkan revs 1870 lämnades det mäktiga västtornet kvar. När den nuvarande kyrkan byggdes 1870-1871 ritad av Fredrik Ekberg försågs tornet med en hjälmformad huv. På denna vilar en lanternin krönt av ett kors. Det stora långhuset i empirestil avslutas bakom korväggen med tresidig sakristia. Utmärkande för interiören är sidoläktarna som ger kyrkorummet ett treskeppigt intryck.
